Aultman College of Nursing and Health Sciences maintains the student records for the following institutions: 

• Aultman Hospital School of Nursing
• Aultman Hospital School of Radiologic Technology
• Aultman Hospital School of Nuclear Medicine
• Aultman College of Nursing and Health Sciences  

We are happy to fulfill transcript requests for any of the above institutions. To request a transcript, please complete the Official Transcript Request Form below.

 You may send in your transcript request in four ways:

• Submit it in person at the College.
• Fax it to 330.580.6654 to the attention of the Registrar.
• Email it to [email protected].
• Mail it to Office of the Registrar, Aultman College, 2600 Sixth Street, S.W., Canton, OH 44710.

There is a $5.00 fee for each transcript request made by anyone not currently enrolled as an Aultman College student.  Please call the College office at 330.363.6347 for payment options.
